If you are able, please print out and fill out patient intake forms. Please note: we are in network with Blue Cross Blue Shield, Health Partners, UCare, Cigna, Medicare, and Medicaid programs. If you do not have an in network insurance or any health insurance, we do offer same day payment and payment plan options. Please contact our office for details about these options.
